As early as 1862 the Old Fashioned was mentioned in Jerry Thomas' Bartenders Guide: How To Mix Drinks. That version though was made with gin, not bourbon or rye. Almost 20 years later the Old Fashioned as we know it today is said to have been invented at The Pendennis Club in Louisville, Kentucky by James E. Pepper, bartender and bourbon maker. He then brought the recipe to the Waldorf-Astoria Hotel bar in New York City. That said, the cocktail was mentioned on February 15, 1880 in The Chicago Daily Tribune so you can’t be entirely sure.
The glass, Kartio, was designed in 1958 by Finnish designer Kaj Franck.
