The word macchiato means stained or marked so a caffè macchiato is an espresso stained with a dash of foamed milk. It became popular in Italy after the invention of the espresso machine in 1901 and is a way to take the edge off the espresso and add a bit of sweetness. Do not get this confused with the latte macchiato which is the complete opposite, milk stained with coffee.
The cup is called Bavero and was designed in 1977 by Achille Castiglioni for Alessi.
